Opportunity Description:

The Portland Veterans Affairs Medical Center – Vancouver Campus is recruiting for a primary care provider for one of their community-based outpatient clinics. The Primary Care Division provides care to more than 63,000 Veterans, with most requiring chronic disease management. The ideal candidates will have previous experience with panel management and managing chronic conditions. We seek a board certified or board eligible Internal Medicine physician.


Primary Care operates under the Patient Aligned Care Teams (PACT) model of care, which consists of one provider and two clinical and one admin support staff, as well as clinical pharmacy and an integrated mental/behavioral health program. The clinic has 14 PACTs (with room to grow), which are individually led by 8 physicians and 6 nurse practitioners. Starting in the Summer 2017, we are opening our doors to a brand new Primary Care building.



VAPORHCS has a close relationship with our academic affiliate, Oregon Health & Science University (OHSU), to which it is physically connected by a sky bridge. The VAPORHCS Primary Care Division has a unique relationship with the Department of Hospital and Specialty Medicine and OHSU, with an Internal Medicine Resident clinic. VAPORHCS has a robust research program, with over $32 million in grant funding. VAPORHCS is a leader in high quality, cost effective clinical care, having been recently recognized as among the best by CMS Hospital Compare, recipient of a 2013 Joint Commission Top Performer Award on Key Quality Measures and a 2013 National Center for Patient Safety Cornerstone Silver Level Award. VAPORHCS is also a Magnet Recognition Program Facility. VAPORHCS and Primary Care are committed to innovation and process improvement and utilizes the VA-TAMMCS (Team-Aim-Map-Measure-Change-Sustain) model to actively study current practices and to develop evidence-based improvements.

Facility Description:

The Vancouver Division is located 12 miles north of Portland VAMC on the Washington State side of the Columbia River. The 50-acre campus houses a variety of clinics, services and programs serving the physical and mental health needs of Veterans, and provides housing and vocational opportunities. Eight psychologists, offer clinical services on-site.

A 72-bed Rehabilitation and Extended Care Center on the Vancouver campus serves the Portland/Vancouver metropolitan area. The Center’s services include extended care rehabilitation, psycho-geriatric care, and general nursing home care. Additionally, outpatient clinic services are offered at this location in a new outpatient clinic facility.
