Call: Practice call shared with other eligible members in the SAMG Department of Family Medicine (Currently 1:24).

Saint Alphonsus Medical Group (SAMG) is a physician led and accountable 450+ provider multispecialty group practice seeking a Family Practitioner (MD/DO) to work full-time at its Lake Hazel clinic. The Family Medicine physicians at the Lake Hazel clinic are dedicated to providing services for a full spectrum of patients to include birth to end of life. The 8,000 square foot clinic is surrounded by an underserved residential area that continues to experience growth and development of new housing. The Lake Hazel clinic has procedure rooms, lab and digital X-Ray services on site, in addition to a fully staffed Urgent Care facility open every day 8am – 8pm. The Lake Hazel clinic utilizes the Cerner PowerChart® for Electronic Medical Record and Computerized Order Entry, so experience with EMR is highly encouraged.
Support: 24/7 CRNA Anesthesia; Level IV Trauma Center; ED; Saint Alphonsus FM clinic on-site; community family medicine clinics; SAHS specialty physicians rotate in outreach, larger system resources.
-Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree
-Successful completion of an accredited ACGME/AOA Family Medicine residency program.
-Board certification through the American Board of Family Medicine (ABFM) or American Osteopathic Board of Family Physicians (AOBFP) (or obtain such certification within 3 years of employment).
-Obtain Idaho medical licensure to practice medicine in all its phases; valid controlled substance registration with Idaho Boards of Pharmacy & DEA
